Annual Forms

Scouts New to Pack 84:

For each Scout joining Pack 84 for the first time, we need a parent/guardian to:

• Register by submitting a paper copy of Scouting America's Youth Registration Form to a Pack 84 leader.

All Families:

The following paperwork is needed each year (even if no information has changed):

• For each Scout:

  • Annual activity permission slip (PDF)

• For each person (registered Scout or parent or grandma or anyone) that will participate in an overnight Pack event:

  • Annual medical release form Part A (PDF)

  • Note: a copy/photo of both the front & back of the appropriate insurance card should be attached to this form.

Forms may be completed by hand or electronically, and delivered either by emailing forms@pack84.org or by submitting them in-person at a den meeting.
